Job description

St HelensNorth Primary Care Network are looking to recruit to a motivated and dynamic PCNUrgent Hub Manager on a fixed term basis to help manage and organise an Urgent Hub which has beencreated to help reduce pressure on GP practices. This role will involve workingacross all 6 practices in the North Primary Care Network.

We are looking for someone who has experience in PrimaryCare, is used to working on EMIS, enjoys patient interaction and is capable ofworking autonomously with minimal supervision.

The successful applicant will work within our friendly Primary CareNetwork multidisciplinary healthcare team and working closely with GP's who also work in the hub. They will also work alongside the wider ARRSteam, and in partnership with the six practices that make up the Primary CareNetwork. This is a temporary role to cover maternity leave and we are looking to recruit someone to cover from February 26 to January 27.

Main duties of the job

Within ourPCN, we have recently created an Urgent Hub model where all six PCN practices areable to book in to an acute hub clinic for same-day patients.

This is a full time role where the successful candidate willbe expected to be based in practice alongside the Urgent Hub. This will involveworking on a rota across multiple practices throughout the week. You will beresponsible for adding clinics to EMIS, booking patients in as the arrive forthe hub and any other reception or PCN duties as and when required. The hub runs from 10am to 6pm, and the successful candidate can work their hours from 8am to 6pm.

In this role, excellent communication skills will be required as there is interaction with patients. You will be expected to build good relationships with both ARRS andPractice staff and ensure the hub runs smoothly at all times. There will alsobe an expectation to help with administrative tasks within the PCN.

Job responsibilities

The successful candidate will be expected to managethe administration of the Urgent Care Hub and provide admin support to theNetwork Manager. They will liaise with Clinical Director and the six PracticeManagers within the Network on a regular basis.

The postholder will have excellentcommunication skills, will be well organised with a proactive approach and haveexcellent knowledge of EMIS.

Principle Responsibilities
  1. 1.Tofully manage the urgent care hub including:
    • EMISclinics
    • staffing
    • reception duties for the Hub in practices working across the Network surgeries.
    • produce reports and provide data for monitoring purposes
    • work with the PCN GP, ANPs, Network Manager, CD and Clinical Lead to make service improvements when required.
  2. 2.Liaisingwith Practice Managers in the Network to collate or disseminate information, including holding teams calls.
  3. 3.Liaisingwith ARRS staff in the Network to collate or disseminate information.
  4. 4.Toprovide administrative support in the procurement of equipment, uniforms, supplies and organising training sessions when needed.
  5. 5.AssistingNetwork Manager when required with the PCN Patient Participation Groups (PPG)for the Network to ensure patients aware of Network activities and feedbacksought on services when appropriate.
  6. 6.ArrangingNetwork meetings, collating the agenda, distributing information and typingminutes where required.
  7. 7.Supportingwith the recruitment process for ARRS Staff.
  8. 8.Completeany training as required for the role.
