Wyre Integrated Network PCN are recruiting for a Paramedic to join their team. The successful candidate will join a team consisting of Pharmacists, Pharmacy technicians, Paramedics, Social prescribers, Care coordinators, GP Assistants and a Physiotherapist supported by a team of GPs and clinical leads all striving to support the needs of the local community.
The PCN Paramedics work autonomouslywithin the community at an advanced level of practice, using their enhancedclinical assessment and treatment skills, to provide first point of contact forpatients presenting in the community or general practice setting.
The successful candidate will be a vital part of our MDT, and will be expect to provide expert advice and collaborate with a range of clinicians to produce the best healthcare outcomes for our patients.

WIN PCN covers 30,900 patients over four practices in the Thornton-Poulton area and uses a network of skilled clinicians to forge relationships with varying healthcare providers and promote collaborative working through the 4 partner practices, providing personalised care for patients through every stage of life.

Responsible for developing, with patients, a continuing management plan for the health of the patient and when appropriate to refer to other clinicians and agencies
The Paramedic will support the network, required to attend same day visits booked in by the surgeries/care homes to support patients in the community.
The successful candidate will work within the multidisciplinary team to deliver the requirements of the QOF, DES and IIF framework as part of the primary care network.
The role will involve independent prescribing within the scope of professional practice.
