A leading healthcare provider in England is seeking a clinical pharmacist. The role involves conducting medication reviews, collaborating with healthcare teams to optimize patient care, and implementing prescribing guidelines. Ideal candidates will have a pharmacy degree, at least 5 years of experience, and be registered with the General Pharmaceutical Council. This position is based in multiple locations across Devon and offers a full-time, permanent contract.
This role is an in person position based at Bow Medical Practice and Wallingbrook Health Group, within the PCN.
Undertake clinical medication reviews with patients and produce recommendations for Nurses and/or GPs on prescribing and monitoring. Review the ongoing need for each medicine, monitoring requirements and support patients in making the best use of their medicines. Make recommendations to GPs for medicine improvement. Identify cohorts at high risk of harm from medicines through pre-prepared practice computer searches, including patient-related and medicine-related risks. Review medicines most associated with unplanned hospital admissions and readmissions and implement changes to reduce prescribing in high-risk groups. Provide medicines support for patients with questions or concerns about their medicines. Reconcile medicines following discharge from hospital, intermediate care and into Care Homes, identifying and rectifying unexplained changes and coordinating with patients and Community Pharmacists to ensure appropriate post-discharge medicines.
The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, structured medication reviews, long-term condition management, transfer of care medicines management, and safer prescribing practices. Support acute prescription requests and address public health and social care needs across all GP Practices within the PCN.
The post holder will perform face-to-face and telephone medication reviews for patients with polypharmacy, especially older people and those with multiple comorbidities.
Provide leadership on quality improvement and clinical audits, and contribute to the management of the Quality and Outcomes Framework. This role supports improvement in quality of care and operational efficiency and may develop into non-medical prescribing if not already held.

Provide medicine information to Practice staff and patients. Answer medicine-related enquiries from GPs, the wider MDT and other network staff.
Support and work alongside the Pharmacy Technician, Prescription Team and Dispensary Team; collaborate with the extended PCN Pharmacy Team to deliver the PCN shared agenda.
PROMOTE PRESCRIBING QUALITY AND SAFETY
Support repeat prescribing policy within each Practice. Manage the repeat prescribing reauthorisation process, review patient requests for repeats, and ensure appropriate monitoring tests are in place.
Analyse, interpret and present medicines data to highlight issues and risks. Undertake clinical audits of prescribing and implement changes in response to MHRA alerts, product withdrawals, and local/national guidance.
Implement local and national guidelines and formulary recommendations. Monitor prescribing against the local health economy’s RAG list and advise on red (hospital-prescribed) or amber (shared care) drugs.
Liaise with ICB Pharmacists and Heads of Medicines Management/Optimisation to exchange best practices and foster collaborative working across the PCN and neighbouring networks. Engage with patients, GP, Nurses, social prescribers, First Contact Physiotherapists, and hospital staff as needed.
