A community healthcare organization in Hayes is looking for a Care Coordinator. The ideal candidate will provide essential support to patients and help coordinate their care effectively. Responsibilities include managing patient care plans, working closely with GP practices, and ensuring patients understand their health needs. This role offers a competitive salary ranging from £25,000 to £33,000 annually for a fixed term of 6 months.
The Confederation, Hillingdon CIC is hiring a Care Coordinator within Primary Care for our Hayes & Harlington PCN in Hayes. We have 1 vacancy on a 6 month fixed term contract for 25 hours a week.
Care Coordinators play an important role within a PCN to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services. They work closely with GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to people and their carers; helping them understand and manage their condition and ensuring changing needs are addressed. This is achieved by bringing together all information about a person7s identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person.
Care Coordinators may provide time, capacity and expertise to support people in preparing for, or following up, clinical conversations, enabling them to be more actively involved in managing their care and making choices that are right for them. They help people improve their quality of life.
We recognise the value that a PCN Care Coordinator can bring to our practices and our aim is to provide exemplary patient care; finding innovative solutions in general practice to deliver the best outcomes to our patients. We are seeking an enthusiastic and forward-thinking PCN Care Coordinator to join the growing team.
The role will be to work within our network of GP Practices to provide a central coordination role for patient care planning.
The Confederation, Hillingdon CIC works with General Practice and other healthcare providers to deliver its vision for Hillingdon to deliver the best primary care outcomes for patients in the whole of London. We are a not-for-profit community interest company. The Confederation develops and supports GP practices, PCNs and Neighbourhoods and their changing needs. We deliver clinical services at scale and in complement to General Practice, and act as the provider representative voice for local General Practice into the wider NHS and other partners. We are part of the NHS but independent, innovative and transformational.
We strive to be an attractive place to work, offering rewarding roles and opportunities to grow to attract and retain great staff that in turn deliver our vision.
