Job description

Position Summary

The Patient safety investigator will provide high‑level support to the Patient Safety Manager, working within the Patient safety team as part of the Clinical Governance structure within the hospital. The successful applicant will be responsible for assisting, guiding and supporting staff members with investigations into patient safety events, to align with The London Clinic's PSIRF policy and plan.

Responsibilities

• Assist staff members with timely, comprehensive investigations of reported patient safety events, coordinating 72‑hour review meetings and allocating incidents to the appropriate area and team.
• Conduct thorough investigations into patient safety incidents using approved methodologies (e.g. systems‑based approaches, human‑factors analysis).
• Produce high‑quality investigation reports with clear findings, recommendations, and improvement actions.
• Work closely with the Patient Safety Manager, discussing patient safety incidents and updating as appropriate through each stage of the process.
• Gather and analyse medical records, staff interviews and other relevant evidence pertaining to all safety incidents.
