A local government authority in Easthampstead is looking for an Intermediate Care Community Support Worker. This part-time role involves providing care and support in a multi-disciplinary team to assist residents recovering from illness. Responsibilities include personal care, monitoring client conditions, and promoting health and independence. We have a great opportunity for an Intermediate Care Community Support Worker on a part-time (25 hours per week), permanent basis.
This role follows a three-week rolling rota, and the attached document provides further details.
You are an enthusiastic, respectful, caring and reliable person who thrives in a fast-paced environment with experience in health and social care, and comfortable working on a three-week fixed rota. You will bring a flexible and adaptable approach, as well as regular access to a car to support your work. You enjoy being part of a large, collaborative team and will be motivated by making a positive difference to the lives of residents in Bracknell.
The Intermediate Care Service is a multi-disciplinary team of health and social care professionals working together to help residents live fuller, more independent lives. We provide fast-paced, responsive support to people in their own homes or residential settings following a hospital stay or crisis in the community. Our service supports individuals for up to six weeks as they recover from illness, surgery or injury, helping them regain confidence and independence on their journey to recovery.
