A primary healthcare provider in Warrington seeks an experienced paramedic to join their multidisciplinary team. This role involves patient assessment, home visits, and supporting chronic disease management. Candidates should have a degree in paramedicine and experience in primary care. A clean driving license and the ability to work across multiple locations are essential. Salary up to £47,810 per annum.
EAST Warrington PCN has an exciting opportunity for an experienced, hardworking and enthusiastic paramedic to join our team of three GP Practices in EAST of Warrington.
You will be able to work both autonomously and as part of a wider team, which will consist of a paramedic team, GPs, and general practice teams. You will be expected to work across all practice locations (on rotation).
There is also a requirement for visiting of patients either in their own home or in a care home environment. Therefore, applicants will have a clean driving license, along with their own car.
As part of our succession planning, the PCNs intention is to develop paramedics to have more of a primary care focus. We are currently developing the scope for this, and therefore it is expected the new recruits will also help to shape this role for the future.
Main duties will include seeing and treating minor ailments, response for any same day requests for acute and long-term conditions, Acute visits, and Care Home visits. You will work with other paramedics across the PCN ensuring patients are seen and treated and given the best care possible.
You will be able to:-
Support the delivery of anticipatory care plans and lead certain community services (e.g. monitoring blood pressure and risk of diabetes);
Clinical Development Plan, (working towards).
This work will see our paramedics transition from the current minor ailment treatment model to a primary care practice model, to working on the following type of pathways:
East Warrington PCN is one of three PCNs within Warrington, Cheshire. We provide primary care services to 51,763 patients, across three GP practices. Birchwood Medical Centre, Fearnhead Cross Medical Centre and Padgate Medical Centre.
With the creation of the 2020/21 Network Direct Enhanced Services (DES) and the introduction of the NHS Additional Role Reimbursement (ARRS) to support primary care, the PCN has recruited staff under the ARRS to support and broaden our primary care team which allows us to provide better care for our patients.
The PCN is headed by our committed Clinical Directors, Strategic Manager, Practice Managers and GP members.
In addition to the PCN our Practices are also part of Quay Healthcare, which is a non-profit company owned by the GP Practices in Warrington. Quay Healthcare provide business and practical support to the PCNs and deliver additional healthcare services to our patients. The PCN’s contract GP provider services through Quay Primary Healthcare CIC who are responsible for providing a number of services on behalf of the PCN; one of these services includes the management of our PCN ARRS staff, therefore this means that you will work alongside members of the PCN and Practice staff on a daily basis, with Quay Healthcare as the lead employer.
Our PCN Paramedic has the following key responsibilities in delivering health services to our patients:
Working as part of the multidisciplinary team, support the delivery, and continue the development, of the on the day patient service in the practice and PCN;
Accurately triage, assess and prioritise patients, including face to face same day and telephone triage, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history;
Proactively respond to referrals and provide assessment and diagnosis at first point of contact, according to patients needs either in the practice or patient homes;
Undertaking home visits for the practice or PCN;
Evaluate the clinical information from examination and history taking and initiate appropriate treatment (including prescribing medications following national or practice/PCN policy, patient group directives, national and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team;
Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes;
Communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings, and treatment choices;
Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCN’s social prescribing service, and where appropriate, other community or voluntary services;
Communicate proactively and effectively with all colleagues across the multidisciplinary team, attending and contributing to meetings as required;
Support in the delivery of enhanced services and other service requirements on behalf of the PCN.
Ensure delivery of best practice in clinical practice, caseload management, education, research and audit to achieve corporate PCN and local population objectives.
Participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through incidents and near-miss events.
Undertake all mandatory training and induction programmes.
Contributе to and embrace the spectrum of clinical governance.
Always maintain a clean, tidy, effective working area.
