A healthcare organization is seeking two motivated Older Adults Community Advanced Support Workers in Sheffield. The role involves providing short-term support to older adults experiencing mental health crises, working closely as part of a multidisciplinary team. Applicants must have a NVQ Level 3 in care or equivalent, along with a driving license and vehicle access. This full-time position offers the chance to make a meaningful impact in the community.

Salary: Band 3 £24625 to £25674 pa pro rata
We are looking for two motivated and enthusiastic people to join the Older Adult Home Treatment Team (OA HTT). The OA HTT operates 7 days a week throughout the year from 8am until 8pm. We provide short term support including holistic assessments and person centred recovery treatment plans for older adults experiencing mental health crisis.
We work with a range of mental health diagnoses including functional illness such as depression, anxiety and psychosis and also those with dementia. We operate as a multidisciplinary team which includes medics, nurses, occupational therapists and psychology. Advanced Support workers will work with the MDT with the aim to prevent hospital admission for service users and support discharge from mental health hospitals.
Candidates must be able to drive and have access to a vehicle.
You will be expected to work both as part of a wider multi-disciplinary team and also working alone within the community.
The job includes working alongside the other disciplines following and implementing plans of care for individual service users.
You will work into service users own homes and care homes in the city.
You will respond to and support our service users at times of crisis.
You will support the weekly MDT and contribute to the reviews of our service users.
You will contribute to the team meetings and service development.
