What you will do:

• Assess and reable clients following discharge from hospital into the community.
• Work independently with clients, families, and carers, traveling throughout the borough of Haringey.
• Maintain records in a timely fashion and contribute to MDT working.
• Conduct regular ADL assessment activities and engage in discussions about appropriate levels of care support.
• Motivate clients to set goals and regain independence.

To be successful in this role, you need to be HCPC registered. A driving licence is essential as you will be traveling throughout Haringey to meet with clients.
