Qualifications

  • NVQ Level 2 in Health and Social Care or equivalent.
  • Current driving licence with vehicle access including business insurance.
  • Knowledge of palliative and end of life care.
  • Hold person-centred conversations and communicate sensitively.
  • Participate in care planning and evaluation within multidisciplinary teams.
  • Undertake risk assessments for safe care delivery.

Responsibilities

  • Plan work and respond to patient condition changes independently.
  • Prioritise care needs including hygiene, nutrition, and hydration.
  • Ensure timely reports to senior staff in the multidisciplinary team.
