Network Navigator Role

The Network Navigator works on an acute mental health ward at Blossom Court, St Ann’s Hospital. The role is full time, in‑person with face‑to‑face working as a minimum expectation. The Network Navigator is part of a wider system of supporting and managing patient access and flow within the North London Mental Health Partnership.

  • Work closely with Discharge Coordinators and ward staff to identify individual patient strengths and barriers to discharge at the earliest opportunity following admission.
  • Organise the system around the patient to join key meetings during admission and ensure timely resolution of barriers to discharge when patients are clinically ready.
  • Facilitate the contribution of carers, a key activity, and coordinate with community mental health, substance misuse teams, voluntary sector organisations, criminal justice staff and housing providers where relevant.
  • Ensure 72‑hour formulation meetings are in place, link with ward staff and complete all actions to facilitate the safe discharge of the in‑patient population as and when they are clinically ready for discharge.
Hours & Working Conditions

Hours of work are Monday to Friday 9 am to 5 pm – five days per week. Please note professional registration is not required for this role.
