Overview

Mental Health Social Worker, Mental Health/Recovery Team – Sanctuary Personnel has a new permanent position available for a Mental Health Social Worker within the Mental Health/Recovery Team to work Full Time based in Bath. The salary for this permanent Mental Health Social Worker job is up to £36,073 per annum.

Responsibilities
  • To work as a social worker/care coordinator in managing a caseload of individuals referred into secondary mental health services for support with their mental health needs.
  • To complete comprehensive individual needs assessment based on a strengths based approach and recovery principles.
  • To co-ordinate appropriate care and support packages of care with clear recovery outcomes. To be accountable for ensuring that the packages are appropriate to the person’s needs and are regularly reviewed.
  • To complete or participate in the assessment of risk, and to produce appropriate risk management strategies.
  • To lead and participate in the review and evaluation of packages of care and support.
  • To liaise with other health and social care professionals, organisations and agencies, including carers and family, housing, probation service, benefits and welfare agencies etc.
  • To maintain case records and general administration related to the caseload in line with department guidance and procedures. To work and understand single case file records.
  • To ensure that legal literacy and a rights based approach is at the forefront of their work.
  • To undertake on occasions the supervision of other health or social care workers, including student supervision.
  • To fully participate in the shared responsibilities and tasks of the team/service, including duty systems and service input, and to participate in multi-disciplinary meetings and assessment clinics.
  • If an AMHP to participate in the AMHP duty rota and where not an AMHP to be prepared to undertake the training to become so.
  • To participate in the team/service and departments staff development programmes and the promotion of specialist skills.
  • To carry out such other duties as are required as are commensurate with the grade of the post.
Requirements
  • A Social Work Qualification or equivalent.
  • Understanding of relevant legislation.
