Job description

We are looking for an experienced Mental Health Practitioner to join our Thurrock PCN for 12 months fixed term and to work across multiple GP practices. This role involves assessment, caseload management, triage and delivery of evidence‑based interventions for children and young people with mild to moderate mental health needs. As part of the NHS long‑term plan, this position supports the realignment of community mental health services with primary care networks, creating a new integrated model of primary and community mental health care.

Primary Care Network (PCN) Goals
  • Have a key co‑ordinating role at the heart of the GP practices and be a fully integrated member of the multi‑disciplinary team for response and management pathways.
  • Provide a combined consultation, advice, triage and liaison function in accordance with agreed protocols.
  • Support children and young people with mild to moderate mental health problems through evidence‑based interventions.

Core Clinical Responsibilities
  • Work as part of a multi‑disciplinary team in a patient‑facing role to assess and manage patients using expert knowledge.
  • Assist in the assessment of new patients and produce individual care plans.
  • Provide specialist clinical support and advice to young people with mental health difficulties.
  • Support patients in engaging with appropriate services and, when necessary, refer to secondary mental health services or signpost to voluntary services and support groups.
  • Educate and involve family members and others in treatment as required.
  • Plan day‑to‑day workloads and follow departmental policies, procedures and guidelines.
  • Collaborate with GP practices to standardise referral processes across the PCN.
  • Triage and receive referrals from within the PCN.
  • Communicate effectively with patients, recognising varying levels of understanding, cultural background and preferred communication methods.

Additional Requirements

The post holder should have experience managing a clinical caseload and delivering therapeutic and low‑intensity interventions for mild to moderate mental health needs, including individual and group support.
