Job summary

A healthcare organization in the UK is looking for a qualified professional to promote a variety of clinical and non-clinical interventions in the Primary Care Network. This role includes care planning, mental health intervention, and contributing to case formulations. The successful candidate will also receive relocation support and be part of a healthcare improvement program.   • Contribute to case formulations within the PCN, incorporating experience and knowledge as a trained professional in mental health.

Key Responsibilities
  • Promote and offer a range of clinical and non‑clinical interventions within the Primary Care Network (PCN).
  • Provide assessment and care planning expertise relevant to your profession.
  • Contribute to the ongoing development and transformation of the care system.
  • Provide effective signposting to a range of health, social, voluntary sector and community‑based resources and services.
  • Work with patients to support shared decision‑making about self‑management and provide brief therapeutic interventions where qualified and appropriate.
  • Operate without the need for formal referral from GPs, including accepting some direct bookings where appropriate.
  • Contribute to case formulations within the PCN, incorporating experience and knowledge as a trained professional in mental health.
  • Support the PCN practices to achieve specific targets relevant to mental health as may be dictated by local and national contracts (DES, NIS, QOF).
  • Participate at practice and network‑based MDTs, share good practice and take part in training activities for other of the MDT.

Relocation scheme: Up to £10,000 for removal and associated expenses subject to terms and conditions.

Successful applicant will be able to complete the Improvement Leaders Programme as part of Quality Improvement ELFT.

This role is part of the Community Mental Health Framework for Adults and Older Adults.
