A health and social care Trust in England is seeking a Mental Health and Wellbeing Coach to enhance community mental health. Responsibilities include providing individual support, managing a caseload, and facilitating goal-setting. The ideal candidate will have a foundation degree or equivalent, with at least a year of coaching experience in mental health. This role involves multi-agency working and aims to empower individuals in their health journey.
The role of the Mental Health and Wellbeing Coach is to work as part of Humber Teaching NHS Foundation Trusts YOURhealth service in the Harthills Primary Care Network locality with the aim of improving the mental health and wellbeing of the local community.
The role will support the Harthills Primary Care Networks and patients within the practices to access low to moderate mental health support through 1-1 support to help them move from surviving to thriving. The aim being to help people feel in control of their health and wellbeing by addressing mental health, general health, wellbeing and inclusion needs using a behaviour change approach and active recovery methodology including the Recovery Star with continuous goal setting and action planning throughout their time with the service.
The role of the Mental Health and Wellbeing Coach is a role integrated as part of the YOURhealth team whilst working clinically with the Primary Care Mental Health Network in Harthills.
