A leading healthcare provider is seeking a Long Term Conditions Nurse in the Greater London area to engage in identifying health needs, facilitating care arrangements, and ensuring continuity of care across multiple agencies. This full-time position offers a salary of up to £55,086 per annum based on experience, supporting nurses in their professional growth with various development options. Ideal candidates will possess NMC registration and experience working with complex health and social needs.
As a Long Term Conditions Nurse, your responsibilities will vary. You will Identify immediate health needs including external services or prescribing requirements that prisoners may have received prior to remand or repatriation onto a wing, Facilitate arrangements between agencies involved in delivering a complex care package, deliver continuity of care across health and social care agencies from courts, to custody and community, and inspire the continuous improvement of MDT working across HMP and healthcare teams.
