Qualifications

Locum Case Manager

As a Case Manager with Network Case Management means building strong and supportive relationships with the individuals and key stakeholders within your network. You will assess, implement and review each client’s needs whilst maintaining a professional and supportive role. Your primary responsibility is supporting the individuals under your case load achieve their goals. This role is a self-employed, locum role and work will be allocated according to your availability and the current caseload.

Responsibilities
  • Regularly corresponding and meeting with your clients to manage their case as appropriate
  • Undertake Initial Needs Assessments and report findings to the necessary stakeholders
  • Liaising with a multi-disciplinary team to plan and facilitate implementation of key strategies which will support with meeting the objectives outlined by the Initial Needs Assessment.
  • Effectively managing your time to ensure you give the necessary time to each client
  • Organising your workload on a daily, monthly and annual basis to ensure you meet expectations as agreed with your General Manager
  • Reporting into your General Manager to update on key timelines and progress against goals for individual clients.
Qualifications
  • Excellent organisational skills, allowing you to successfully support a caseload of individuals in a variety of areas of life.
  • Excellent written and verbal communication skills including report writing skills
  • Current registration with HCPC as an Occupational Therapist OR current registration with NMC as a Registered Nurse. Currently practicing Solicitors can be considered for this role also, with relevant experience.
  • Minimum 5 years practice as a Professional Registrant in your industry
  • Current registration or willingness to register with IRCM
  • Minimum 2 years’ experience in a role as a Case Manager supporting clients with Brain Injury or Complex Needs
  • Ability to undertake an Enhanced DBS Check (Adults and Children as appropriate.)
  • A thorough understanding of key legislation relevant to successful case management including but not limited to Mental Capacity Act 2005, Health and Social Care Act 2008 and Care Act 2014.
  • Member of BABICM, VRA or CSMUK
