A local healthcare provider in Crewe is seeking an experienced Lead Practice Nurse/Nurse Manager. This role involves leading the nursing team, managing clinical policies, and delivering high-quality patient care. The post holder is responsible for ensuring the delivery ofsafe and effective nursing care to the whole practice population. They willlead and manage the nursing team, working closely with the Partners and thepractice management team to deliver the practice priorities as well assupporting the review and delivery of clinical policy and procedure.
Clinically, the focus of the role is the delivery ofevidence-based practice for patients with long-term conditions and management,health promotion and preventative nursing interventions to all patients. As anautonomous practitioner the lead practice nurse is responsible for the nursingcare delivered, demonstrating critical thinking and skills in clinicaldecision-making.
An opportunity has arisen in South Cheshire for anexperienced Nurse Manager post. Located in a purpose-built health centre in asemi-rural location, you would be joining a positive, large, well organised andfriendly practice.
We are looking to recruit an enthusiastic and committed LeadNurse with the desire to deliver a high quality of care and to continue thedevelopment of our well established and well regarded practice. We have apatient list size of over 21,000.
Our clinical team comprises five GP Partners, eight SalariedGPs, three Advanced Nurse/Clinical Practitioners, seven Practice Nurses plus aHCA, GPA and a health and wellbeing coach as part of the wider nursing team.

Key Responsibilities
Clinical practice
Assess, plan, develop, implement,provide and evaluate treatment programmes including, but not limited to:
- Healthpromotion and well-being plus individual plans for patients with/at risk of along term condition
- Manage and treat long-term conditions, in line withnational and local policies and practice needs
- Review medication fortherapeutic effectiveness, in accordance with evidence based practice andnational/practice protocols, and work with patients to support adherence toprescribed treatments
- Prioritise health problems, intervening appropriately, toassist the patient in complex, urgent or emergency situations, includinginitiation of effective emergency care
- Support and manage health needs of womenpresenting for family planning and cervical cytology consultations
- Implementand participate in vaccination and immunisation programmes for both adults andchildren including patients travelling abroad
- Meet the needs of patientspresenting for wound care Diagnose and treat minor illnesses Co-ordinate clinicalcare pathways
Communication
- Demonstrate sensitive communication styles to ensurepatients are fully informed and consent to treatment
- Communicate effectivelywith patients and carers, recognising the need for alternative methods ofcommunication to overcome different levels of understanding, culturalbackground and preferred ways of communicating, including those receiving badnews
- Act as an advocate when representing patients and colleagues
- Participatein practice team meetings, delivering the nursing agenda and run nursing teammeetings
- Produce written documents that evidence the contribution of thenursing team to the practice priorities
Delivering a quality service
- Recogniseand work within own competence and professional code of conduct as regulated bythe NMC
- Produce accurate, contemporaneous and complete records of patientconsultation, consistent with legislation, policies and procedures
- Prioritise,organise and manage own and others workload in a manner that maintains andpromotes quality
- Deliver care as an individual and team according to NSF, NICEguidelines and evidence-based care, assessing effectiveness of care deliverythrough peer review, benchmarking and formal evaluation and leading on themaintenance of quality governance systems for the nursing team
- Implement andreview the application of evidence-based practice in nursing
- Utilise the auditcycle as a means of evaluating the quality of the work of self and the team,implementing improvements where required
- Evaluate patients response to healthcare provision and the effectiveness of care
- Lead and participate in sharedlearning across the practice and wider organisation and assess the impact ofpolicy implementation on care delivery
- Monitor the performance of the GPN teamin accordance with local policies
- Work within policies regarding familyviolence, vulnerable children and adults, substance abuse and addictivebehaviour, and refer as appropriate
- Interpret national strategies and policiesinto local implementation strategies that are aligned to the values and cultureof general practice
Leadership personal and people development
- Takeresponsibility for own development, learning and performance includingparticipating in clinical supervision and acting as a positive role model
- Support the development of others in order to maximise staff potential

Management of risk

- Ensure safe storage, rotation and disposal ofvaccines and drugs
- Act as a role model to support members of the nursing teamto undertake mandatory and statutory training requirements
- Applyinfection-control measures with in the practice according to local and nationalguidelines
- Participate in the local implementation strategies that are alignedto the values and culture of general practice
- Usetechnology and appropriate software packages as an aid to day to day operations
- Review and process data using accurate read codes in order to ensure easyaccurate information retrieval for monitoring and audit processes
- Adhere tostatutory and practice requirements such as confidentiality and Freedom ofInformation Act
- Collate, analyse and present clinical data and information tothe team using appropriate charts and/or graphs to enhance care
Education,Learning and development
- Act as mentor for more junior staff and students,assessing competence against set standards
- Co-ordinate education and learningand development within the nursing team
- Disseminate learning and information toother team members in order to share good practice and inform others aboutcurrent and future developments (eg courses and conferences)
- Provide aneducational role to patients, carers, families and colleagues in an environmentthat facilitates learning
- Assess own learning needs and undertake learning asappropriate
- Undertake all mandatory training required for the role

The post holder should respect patient confidentiality atall times and not divulge patient information unless sanctioned by therequirements of the role. All information concerning patients, staff and thepractice must be treated as strictly confidential at all times.
