Main Responsibilities

  • Developing and implementing care plans for patients.
  • Conducting physical health monitoring, including examinations and investigations as needed.
  • Liaising with GPs and other professionals or teams within and outside the Trust.
  • Maintaining accurate and clear record‑keeping, preparing reports, letters, discharge notifications and summaries, ensuring all relevant information is recorded in the Trust’s electronic notes system.
  • Participating in service evaluation, audit, research and quality improvement activities.
  • Engaging in service development and clinical governance activities.
  • Participating in academic activities, including teaching, case presentations and reflective practice sessions.
  • Receiving regular supervision and support, with access to study leave and professional development opportunities.
