Job description
Responsibilities

We are looking for a confident and experienced practitioner to work full-time 37.5 hours per week within the Staffordshire Reconnect Service. The Reconnect service sits within our wider Health and Justice service, supporting people with needs and vulnerabilities in all aspects of the criminal justice system. Reconnect reaches into 5 of the prisons within Staffordshire, undertaking holistic needs assessments and engagement work in order to support them on release from prison to get their health and social care needs met within the community and reduce the risk of reoffending. Reconnect sits within a national network of reconnect services, and works both within and outside of the prison estate. The role will include the supervision of junior staff, assessing complex clients, triaging referrals for suitability to the service and supporting the day to day operational leadership of the service. Flexible working arrangement would be considered, with the potential to increase the hours to meet changes in service provision.

The post holder will be responsible for working in the all age Integrated Offender Health (IOH) Services, with the flexibility of working across Staffordshire, dependent on skill and service need. The Practitioner will not be required to provide direct treatment to individuals, but will be required to complete assessments of individuals, often complex, needs. The assessments will then be utilised to make appropriate referrals for treatment and support. Whilst also ensuring criminal justice partners are notified of specific health requirements and vulnerabilities of an individual which can be taken into account when decisions about charging and sentencing are made.

As a Trust we remain bold and ambitious with plans for both service and system collaborative transformation over the coming years to improve the health and wellbeing of our local people through high quality care.
