Qualifications

  • Able to work across a 7-day rota, including one weekend every four weeks.
  • A full UK driving licence and access to transport is essential.

Responsibilities

  • Oversee Care Act assessments and ensure strengths-based interventions.
  • Work collaboratively with healthcare partners for integrated service delivery.
