A local council in Manchester is looking for experienced Social Workers to join their fast-paced community service team. The role involves assessment, support planning, and safeguarding investigations for citizens both in homes and care placements. Qualified candidates must possess a Social Work Qualification and registration with Social Work England. This is a full-time position with various benefits including generous holiday leave and a pension plan.

Additional Payments: 6.7% Flexibility Payment, Essential Car User Allowance

We are looking for motivated, confident and experienced Social Workers in a fast paced MDT team working within the HDTA Community Service for North Manchester.
This is a busy and focused team which involves critical thinking and the ability to manage risk and need in partnership with Health colleagues within the HDTA service. The role involves assessment and support planning for citizens both in their own homes and in some cases 24hr care placements. Social workers will be required to undertake Mental Capacity Act Assessments and coordinate Best Interest Decisions. Social workers will be required to lead on Safeguarding Investigations. Social workers will be required to be part of the team duty rota.
The role will suit Social Workers able to work at pace in collaboration with others to support citizens discharged from Hospital under the HDTA service.
The ideal candidate will be a qualified, experienced and confident social worker able to work closely with social care and health colleagues and able to complete comprehensive Strengths based Assessments for people who may be in need of long term care.
Candidates would benefit from strong communication skills, the ability to challenge in the best interest of citizens and able to manage a fast paced caseload. Successful candidates will have access to regular formal and informal supervision and will be confident in working within a wide MDT of health and social care staff.
Candidates would benefit from strong assessment skills and experience of collaborative working with health colleagues.
