Overview

Farleigh Hospice is a high performing organisation providing specialist palliative and end of life care for people with life limiting illness in the Mid Essex region. We are looking for a Care Coordinator to join our team. The post will develop relationships both in-house and with external organisations, to ensure that the patient has access to the most appropriate care environment for their holistic needs. The candidate should manage timescales sensitively and have a good understanding of GDPR and confidentiality. You will be a team player, motivated, forward thinking and a talented individual interested in delivering excellent specialist palliative care within the Hospice Rapid Access Service team.

Responsibilities
  • Lead the procurement of nursing home placements and/or domiciliary care for patients clinically assessed for the Hospice Rapid Access Service.
  • Develop and maintain relationships with internal teams and external organisations to secure appropriate care environments.
  • Manage timescales sensitively to ensure timely access to care.
