Overview

We are looking for an Integrated Care Worker to join our Ely and surrounding villages Community Nursing Team. We have an exciting opportunity to onboard a new member to our Nursing team, working with us, you will be fully supported to gain new skills and experience needed to deliver high quality care to patients in their own home or residential setting in and around the area. We provide a service seven days a week, working closely with other community services and Primary Care Networks to provide high standard holistic care to the population of Cambridgeshire. You will have the opportunity to develop yourself, with extensive learning and development opportunities. Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Responsibilities
  • As an Health Care Assistant you will work closely with the nursing team to follow a plan of care in deliver high quality, best practice care to adults with a range of complex health needs within their own homes. We are looking for caring, responsive and diverse people who share our passion to deliver our service.
  • To undertake patients prescribed treatment/care plans, i.e., venepuncture, insulin administration and wound care, and report findings to a qualified professional in line with competency framework.
  • Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy.
  • Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals.
  • Work under guidance on specified tasks, taking direction from registered and non-registered staff as appropriate, with regular clinical support and supervision by a qualified practitioner.
  • Promote patient independence and self-management. Provide and maintain a safe, comfortable and dignified environment for all patients/clients and carers. Practice the principles and philosophy of individualised patient care in accordance with Trust Policy, procedures and regulatory bodys guidelines.
  • Maintain appropriate and timely documentation of each patient contact (ensuring these are recorded on SystmOne within 24 hours).
  • Plan and prioritise own visits to delegated patients (and ensure these are documented on SystmOne).

  • Experience of working in the community.
  • Experience of working within health or social care.
  • Experience of caring for and supporting adults and older people.
  • Demonstrate a commitment to high quality care & service provision.
  • Good communication skills.
  • The ability to work flexibly and as part of a team, as well as individually.
