Job summary

This is an exciting opportunity to be at the forefront of addressing health inequalities across Darlington. The post holder will use their experience from working in the community with individuals and groups combined with excellent communication skills to engage and deliver support for vulnerable populations who are not utilizing health care services.

Main duties of the job

The purpose of the role is to support the Health Squad Programme in the development, mobilisation and delivery of the new Darlington Health Squad programme and line manage a team of Advanced Practitioners. The post holder will support management, line manage and deliver interventions, including health checks, with clients.

The role will be at the forefront of addressing health inequalities across Darlington working in partnership with a variety of other key teams will be vital to the success of the programme.

The post holder will use a population health management approach to ensure targeted and prioritised support protecting the most vulnerable populations and improving health and wellbeing across County Durham.

Engage with clients and offering them support and brief intervention advice and offering physical health checks, including finger prick sampling, BP readings, body measurements, and undertaking mental wellbeing assessment tools.

Support clients to understand and manage their health and wellbeing through behaviour and lifestyle changes assisting them to achieve their goals, manage their condition and undertake prehabilitation.

Experience (Essential)
  • Experience of line managing staff or volunteers, undertaking supervision and appraisals
  • Experience of working in the community or voluntary sector
  • Experience of successfully engaging and working with both individuals and groups
  • Experience of evaluating projects or services, using a range of techniques
  • Experience of supporting people and their families in a related role (including unpaid)
  • Experience of effective team working, with the ability to use own initiative
  • Experience of partnership/collaborative working and of building relationships across a variety of settings
Experience (Desirable)
  • Experience of multi sector working
Special Requirements (Essential)
  • The ability to regularly travel independently across County Durham within the working day
  • Flexibility including occasional evening or weekend work.
Special Skills & Knowledge (Essential)
  • A sound understanding of the tools and models used in the line management of staff with the ability to demonstrate an effective collaborative and supportive management style
  • Knowledge of health and lifestyle issues relating to adults (gained through practical experience and/or a health related qualification)
  • Good understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities individuals, their families and carers.
  • A substantial working knowledge of supporting clients with long term health conditions utilising a personalised care approach
  • Excellent communication and interpersonal skills with the ability to listen, influence, negotiate and motivate individuals in relation to health related behaviours
  • Ability to act upon own initiative, respond to changing situations with excellent organisational and time management skills
  • Ability to handle sensitive data with confidentiality.
  • Good sound IT skills with experience in a range of Office packages.

Qualifications (Essential)
  • Educated to degree level or equivalent experience in a relevant discipline e.g. Social Work, Nutrition, Public Health, Mental Health, Physiotherapy, Occupational Therapy etc.
  • Level 3 brief intervention, Nutrition and Weight Management / or equivalent
  • Good overall topic based training covering a range of wellbeing disciplines
Qualifications (Desirable)
  • Health Trainer qualification (City and Guilds Level 3) with core competencies completed
  • Level 2 qualification in mental health
  • First Aid Qualification
  • Health Check Training (within 3 months of appointment)
