A health care provider in Liverpool is seeking an experienced Health Practitioner Assistant/Health Care Assistant. The role involves delivering support to patients in a community setting, ensuring high standards of care aligned with organizational values. We have a number of posts working around South Sefton and Liverpool providing care and support for patients who require support with keeping them at home rather than admitting them to hospital or patients who have been in hospital and require support to ensure that their assessments for care are made in their own home rather than hospital.

The successful candidate will have the opportunity to learn, develop and practice a variety of skills, as well as being a valued member of our exceptional multi-disciplinary team.
Communicate information to multi-disciplinary team and other agencies as appropriate. Undertake further training to gain skills in other areas.

The post holder is responsible for the delivery of clinical care and support under the supervision of a registered nurse to a defined population. Supported by a nurse practitioner, they will deliver care within the boundaries of their role. They will work effectively and collaboratively within the team to meet the needs of patients, support the delivery of policy and procedures and provide clinical leadership.
