Overview

An exciting opportunity has arisen within the Health and Justice Reconnect team for the role of Health and Justice Support, Time, Recovery Worker based in Staffordshire. The role may include visits to prisons to support people prior to release throughout Staffordshire including HMP /YOI Brinsford, HMP Dovegate, HMP/YOI Drake Hall, HMP Featherstone, HMP Oakwood, HMP Stafford, HMP/YOI Swinfen Hall and YOI Werrington. A significant amount of time will be spent supporting Service Users in the community post release from prison or immigration removal centre.

Responsibilities
  • Identify people who have substance misuse, mental health, learning disability or other vulnerabilities when they first come onto contact with the criminal justice system or Immigration Removal Centre.
  • Identify unmet need and vulnerabilities that may lead to or exacerbate offending behaviour, offer onward referral and signposting as appropriate.
  • Liaise across agencies to support onward planning and intervention to address identified needs.
  • Work in Police Custody Suites, Courts, Prisons, Immigration Removal Centre and Community; offer assessments to individuals within the Criminal Justice system or Immigration System. This is an all-ages service and involves working with people from the age of 10 years upwards.
  • Work with service users referred to Health & Justice and the individual’s carers to promote self-determination, choice and autonomy.
  • Effectively establish a therapeutic relationship with service users and relatives / carer.
  • Participate in setting quality standards, including auditing, monitoring and reviewing practice in line with current clinical guidance, practice and policy.
  • Assess patients’ individual care needs, agree and refer on to appropriate agency; involve the individual in planning and prioritising their activity where possible.
  • Provide short-term intervention to encourage engagement with services and attendance at initial appointments.
  • Complete and contribute to robust risk assessments and risk management plans; promote positive risk-taking where appropriate to improve quality of life and maximise independence.
  • Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
  • Utilise agreed outcome measures to review the effectiveness of processes, as part of the Health & Justice Service.
  • Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
  • Contribute to working within an award-winning NHS Trust, supporting innovative approaches to improve life for local communities.
