Job summary

Windsor Primary Care Network is a group of 6 GP Practices working together with a range of local providers to offer more personalised and coordinated health and social care to the 70,000 patient population of Windsor in Berkshire. Provide one-to-one coaching to help patients identify goals and take steps towards improving their health and wellbeing.

About the Organisation

Windsor Primary Care Network has been successfully delivering care to the population of Windsor for 4 years. We have a large team of both clinical and non‑clinical staff.

Responsibilities
  • Provide one-to-one coaching to help patients identify goals and take steps towards improving their health and wellbeing.
  • Support patients with lifestyle change areas such as weight management, physical activity, healthy eating, stress management, sleep, and smoking cessation.
  • Use behaviour change approaches including motivational interviewing, active listening, and goal setting to encourage patient confidence and self‑management.
  • Deliver coaching remotely or face‑to‑face across primary care sites.
  • Connect individuals with peer support, self‑management tools, and community activities to build confidence, resilience, and social connection.
  • While each Health and Well‑being Coach becomes embedded in up to 3 Practices, they frequently enjoy a mutually supportive joint approach to work with other members of the 13‑strong PCN Health and Well‑being Team known as the PAC Team.
  • The team benefits from both practice and PCN supervision and training.
