Job description
Overview

Our multidisciplinary teams provide a 7‑day, 365‑day service, operating from 8 am to 8 pm. Patients discharged from hospital into Home First receive step‑down intermediate care delivered in their own homes to support recovery and monitor health in the early days post‑discharge, helping prevent readmission.

Responsibilities
  • Deliver personal, health and therapy care for patients across Sussex.
  • Assist multidisciplinary colleagues in the assessment and provision of care and reablement as prescribed within personalised care plans.
  • Perform simple clinical and therapeutic procedures such as taking physical observations, venepuncture, administering medications, simple wound care, rehabilitation programmes and equipment provision.
  • Observe changes in patients’ status, provide verbal updates and maintain accurate records.
  • Support people in achieving greater independence in daily life.
Requirements
  • Full UK driving licence and access to a vehicle.
  • Ability to work flexibly across sites, including patients’ homes and community hospitals.
  • Willingness to learn; previous experience not required – training will be provided on the job.
