A community healthcare provider in Newcastle upon Tyne seeks a General Practitioner to support rehabilitation services for older patients. This role involves managing a clinical caseload, working collaboratively with multidisciplinary teams, and leading clinical governance. Ideal candidates must be fully qualified, possess leadership experience, and demonstrate commitment to professional development. This position offers flexibility with sessions primarily on Monday and Thursday mornings, contributing to a dynamic healthcare environment.
Weare looking for an existing General Practitioner to work with the Community andOlder Peoples Medicine Services, two sessions a week, to support a clinicalcaseload of patients undergoing rehabilitation alongside a multidisciplinaryteam to promote recovery of function and a return to independent living wherepossible.
The postholder will work as an integral member of the intermediate care team as the named clinician for a cohort of 10-15 patients. They will be expected to oversee and coordinate a Comprehensive Geriatric Assessment for patients who have either stepped up from the community to avoid acute hospital admission or are clinically stable following and acute admission.
It is expected that the post holder will provide clinical sessions within the intermediate care unit/rehabilitation ward for 2 sessions per week. Ideally sessions will be worked in the morning on a Monday and Thursday.

TheGP in intermediate care will have the following responsibilities:
Clinical
Tomanage a clinical caseload of medically stable patients within intermediatecare/rehabilitation ward
Towork closely with a Multidisciplinary team to provide individualised care topatients promoting return to independence via a Comprehensive GeriatricAssessment
Workclosely with the hospital Older Peoples Medicine team and community frailtyteams
Adegree of flexibility will be required in keeping with the variability anddemands of the case mix
Managementand Leadership
Activeinvolvement in clinical governance process within intermediatecare/rehabilitation ward
Providevisible leadership to promote a fair and supportive culture responsive to theneeds of the diversity of our patients, visitors and workforce.
Research
Nospecific commitments but enthusiasm for audit and quality improvement would bewelcomed and research opportunities exist within Older peoples medicine.
Teaching
Therewill be no formal teaching required within the post. There may be occasionalstudents working in the clinical areas. There is an expectation the ethos ofthe team will support ongoing learning and development and therefore informalclinical teaching to MDT members around individual patients and cases will beexpected.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
