A healthcare provider in Bournemouth is seeking a part-time Frailty Nurse to join their Assessment and Care Team. The role involves providing proactive support to frail patients in the community, conducting health assessments, and facilitating independence at home. Candidates must have Adult Nursing Registration and experience working with older patients. This position is essential for reducing hospital admissions.
We are seeking a Frailty Nurse to work 22.5 hours per week across Monday, Tuesday and Wednesday as part of our well-established Assessment and Care Team (AC Team).
Our AC Team support frail and housebound patients registered at our four practices as well as residents in Care Homes.
To provide a proactive approach for the populations of the four Practices in the CentralBournemouth Primary Care Network (PCN), deemed to be at risk of admission, with the aimof providing the support to remain independent at home and to reduce unnecessary hospitaladmissions.
We are a Primary Care Network comprised of four GP surgeries with 50,000 patients.
Our PCN-wide teams and services are based at East Way Clinic which operates as our network hub.
The role is intended to be complementary to the existing general practice and communityservices, and to address a perceived gap in the support of people who may not haveimmediate nursing, medical and therapeutic needs but would benefit from regularassessment. The purpose of which would be to screen for common age-related conditions,provide advice on accessing preventative services and build confidence with self-management.
