Overview

An exciting opportunity has arisen for an enthusiastic, well-motivated professional to specialise in end-of-life care.

St Luke's is delivering on its vision of a Hospice without Walls with the support of its End-of-Life Urgent Care Team. As a Registered Nurse for the End-of-Life Urgent Care Service, you will assist in supporting community-based end-of-life (EOL) patients in meeting their preferred place of care/death and prevent inappropriate admission to secondary care.

Main duties
  • Work autonomously as a Band 5 Nurse and be responsible for ongoing holistic assessments and planning of care to meet patients’ practical, psychological, spiritual, and social needs.
  • Collaborate with primary and secondary care health professionals and other Hospice colleagues; strong communication skills are essential.
  • Join a friendly, dynamic, and progressive organisation where innovation and a can-do ethos are championed; contribute to the specialist End-of-Life Urgent Care service.
