Overview

Surrey Downs Health & Care invites applications for a Band 7 Community Nursing Team Lead role (full-time 37.5 hours/week), permanent, based at Leatherhead Community Hospital. The successful candidate will join the Leatherhead PCN team and work within a multidisciplinary group across the PCN and Surrey Downs Health and Care. Responsibilities

Summary of Band 7 roles for Leatherhead PCN: District Nurse Team Lead and Community Nursing Team Lead are Band 7 leadership positions responsible for delivering high-quality, person-centred care in the community. Core duties include leading and managing a community nursing team, over viewing assessment, planning and implementation of care, staff development, maintaining professional standards, contributing to clinical governance and risk management, promoting integrated care, workforce planning, change management, managing resources and participating in KPIs and audits. Roles emphasise respect, collaboration, safeguarding and high delivery of care to patients and staff.

  • Provide leadership within the District Nursing Team; assess patient needs and develop, implement and evaluate a programme of care within agreed standards.
  • Lead educational programmes for basic, post-basic and in-service training in liaison with Learning and Development.
  • Lead and facilitate patient-focused, innovative research-based practice aligned with SDH&C objectives.
  • Be accountable for assessments, care planning and prescribing appropriate for the patient group; involve, support, inform and educate family/carers.
  • Plan staffing and skill mix to ensure adequate cover; supervise and manage resources and budgets within remit.
  • Provide specialist knowledge to influence the SDH&C strategic agenda; support workforce planning and change management.
  • Maintain clear, signed, contemporaneous records; participate in clinical governance, risk management, audits and clinical audits.
  • Identify workforce planning issues; participate in recruitment, selection and retention of clinical staff; provide support and learning environment for students (pre and post-registration).
  • Ensure adherence to policies, procedures and care standards; contribute to research and development opportunities; resolve complaints within the clinical specialty as required.
  • Collaborate across agencies to maximise effectiveness of care; protect vulnerable individuals; promote clinical governance and risk management strategies; maintain professional integrity per NMC guidelines.
  • Undertake other duties consistent with the post and ensure timely data entry for the post and corporate teams.
  • This post holder may be required to move base temporarily or permanently to meet service needs.
