Working closely with the Digital and Transformation Lead supporting the digital agenda for Primary Care Network.s

Main duties of the job

Work closely with the Digital andTransformation Lead and PCN Clinical Directors.

Support the digital agenda for PCNs.

Support the design, implementation andmonitoring of PCN projects and initiatives.

Support the development of excellentrelationships across the PCN

ChesterSouth Primary Care Network is a collection of four GP practices workingtogether at scale to support the delivery of the NHS Long Term Plan. Ournetwork has been created to facilitate delivery of more integrated care to ourpatient population of approx. 37,000. We aim to better respond to need of localpopulations and build relationships across our care community

  • Assist with improving the adoption of new technologyto enhance patient access and experience.
  • Undertake Digital Clinical Safety training,essential and intermediate level.
  • Support population health management improvementsin care quality and PCN operational efficiency
  • Look at evidence and information availableto support understanding of challenges and in collaboration with the DTLidentify possible solutions and track impact of change.
  • Support Quality Improvement projects todeliver PCN priorities.
  • PCN data submissions and PCN IT requirements.
  • Management of PCN Network Drive
  • To assist with operational projectmanagement processes, developing reports and action logs.
  • Gather data from GP clinical systems EMIS and undertake initial analysis.
  • Develop templates and reports on the GPclinical system.
  • Assist in the collection of information forperformance reporting on workstreams to support information and data flow.

To work with the Clinical Directors and DTLto ensure the smooth and efficient operation of the PCN, including support inpreparing meeting papers, coordination of stakeholder meetings and supportingthe dissemination of key publications to the network members.

Assistthe DTL in administrative duties relating to PCN activities and workstreams.Ensuring all administration paperwork, including the taking of meeting minutes,are drafted promptly, and are distributed / uploaded where appropriate.

Supportthe PCN by planning and organising meetings, including planning staff andpatient engagement events as directed, providing all necessary supportingmaterial.

Dealwith any queries / requests from PCN members accurately and within a timelymanner and to deadlines set.

Ensurethere is effective communication and dissemination of information betweenpractices, Practice Managers and wider stakeholders as appropriate.

Supportthe team in understanding the current guidance for PCNs from NHSEI and keepinginformed of any new guidance or significant news.

To raise awareness of the operational andadministrative processes within the PCN.

To be a proactive team player, be able tomanage their own time and workload and work under own initiative.
