A leading mental health service provider in Ashton-under-Lyne is seeking a DIALOG+ Implementation Clinical Lead to enhance care planning practices. The role involves clinical leadership, supporting the use of DIALOG+ across community services and inpatient wards. The DIALOG+ Implementation Clinical Lead role will work collaboratively with department managers and other members of the leadership team to champion and embed a culture of best practice in the use of DIALOG+ to support care planning in the Trust.
The post holder will act as a subject lead, supporting implementation of DIALOG+ across the Trust community services and in-patient wards – one role will support community and one role will support the wards to ensure a consistent and effective use of DIALOG+ is in place.
The post-holder will work collaboratively with the borough department leads, Head of Quality
and Service Managers to provide clinical oversight, leadership and support to support the
implementation of DIALOG+ across PCFT services. The post-holder will be expected to take a
role in delivering the implementation plans successfully. The post holder will also require an
understanding of the national move away from the Care Planning Approach framework towards
a personalised and collaborative care planning approach
To be a clinical leader and role model with active participation in the implementation of DIALOG+ in the community and inpatient areas.
To participate in audit activities that support DIALOG+ assurance and oversight.
The postholder will act as a primary contact point for all staff who are trained to deliver the DIALOG+ training, and therefore they will be expected to understand the training and specialist considerations for each service line. The postholder will be supported in this by
attending the train-the-trainer sessions, facilitated by ELFT.
The postholder may be required to lead training sessions in exceptional circumstances if a respective team trainer is unavailable.
