It is an exciting time to join the REaCH service. We are passionate about delivering high quality and person-centred care and support to adults aged 18 and over living in Stockport. We work in partnership with Adult Social Care and NHS colleagues to deliver a joined-up approach to the delivery of care and support to the people we support. The Lead team are co-located with the Discharge to Assess Team in Regent House.
The 'Lead' team are very much at the heart of the service and are responsible for completing initial assessments and developing support plans and risk assessments, which are reviewed weekly or as needs change. The team are also responsible for arranging minor equipment, aids and adaptations and work closely with therapists in the development of moving and handling plans and risk assessments. The Lead team ensure that a reablement ethos is instilled from the outset of a person starting to receive our support, to maximise their independence and support them to regain or relearn skills and confidence linked to activities of daily living. Our ultimate aim is to wherever possible support the person to become independent so that they do not require long term support, but where the need for this is identified, the Leads will refer the person on for a Care Act assessment.
Stockport Council holds a Visa sponsorship licence but this can only be used for certain roles and this campaign does not currently qualify.
