Job description

We know that service users recover quickly at home, and you will be an integral component for ensuring that this is done in a safe and specific way as an alternative to hospital admission, working alongside the wider MDT.

As a Band 6 Nurse in the CRHT, you will provide urgent and emergency triage, advice and assessment for people in a mental health crisis. You will undertake home treatment visits as indicated by the MDT, delivering highly skilled nursing care, including care planning, biopsychosocial interventions, clinical risk assessment and management, and promoting recovery and inclusion. You will work autonomously, attend MDT meetings, undertake gate‑keeping assessments for inpatient and crisis house admissions, collaborate with the crisis house to lead on admission care, and facilitate early discharge where clinically indicated. You will also provide clinical leadership and supervision for junior staff.

Responsibilities
  • Provide a clinical service that is service‑user focused, accessible and responsive 24 hours, 7 days a week.
  • Triage all mental health referrals to determine priority and appropriate clinical pathway.
  • Deliver highly skilled nursing care, including full mental health assessment, care planning and delivery, biopsychosocial interventions, and clinical risk management.
  • Recommend and refer, where treatment is indicated, to the most appropriate team or service.
  • Conduct gate‑keeping assessments for inpatient admissions and crisis house admissions.
  • Collaborate with the crisis house to lead on admission care.
  • Facilitate early discharge from inpatient wards where clinically indicated.
  • Provide home‑based treatment interventions for all clients, ensuring best practice is implemented.
  • Participate in Mental Health Act assessments where appropriate to explore alternatives to formal hospital admission.
