We know that service users recover quickly at home, and you will be an integral component for ensuring that this is done in a safe and specific way as an alternative to hospital admission, working alongside the wider MDT.
As a Band 6 Nurse in the CRHT, you will be responsible for providing urgent and emergency triage, advice and assessment for people in a mental health crisis. You will also be responsible for undertaking home treatment visits as indicated by the MDT. This will include the provision and delivery of highly skilled nursing care, including care planning and care delivery, biopsychosocial interventions, clinical risk assessment and management, and promoting recovery and inclusion.
You will be required to work autonomously, understand capacity and flow of the service and the organisation, attend MDT meetings, undertake gatekeeping assessments for inpatient admissions and crisis house admissions, work collaborative with the crisis house to lead on care for admissions as a service and facilitate early discharge from the inpatient wards where clinically indicated. As a Band 6 Nurse in the CRHT, you will also provide clinical leadership and supervision for junior staff.
