We are looking for a dynamic, compassionate, caring and motivated Elderly Care Consultant to join our friendly and enthusiastic team. The successful candidate will join nineteen substantive consultants in providing the highest quality care possible for older patients across our hospital sites and the respective communities of Wakefield and North Kirklees.

Main duties of the job

Regular audit, data collection and quality improvement work to ensure ongoing improvements in the care of frail older people occurs

Maintain overview of all ward patients

Actively participating and facilitating discharges on the ward

Support and supervise junior doctors on the ward

Liaise with Same Day Emergency Care ANPs if necessary

Contribute in the electronic and verbal handover of patients

Job responsibilities

This post is offered on a full time (10 PA) basis although less than full time applications will be considered, and the department has a mixture of full and part time consultants to ensure a supportive environment for all. All our consultants work cross site, but the successful applicant will work within the organization predominantly either at Pinderfields or Dewsbury and District Hospital. The department last year saw 9,403 admissions, saw 654 new patients in clinics and carried out 844 follow up appointments.

The Elderly Medicine Department within the Trust consists of 17 substantive Consultants. The Department is supported by Associate Specialist, Specialist Registrars, Core Medical Trainees, VTS Trainees, FY2s and FY1s.

This post is associated with a number of different specialty interests including Community, Ortho-Geriatrics, Parkinsons and acute frailty.

This post includes on-call responsibilities as part of a dedicated Elderly Care on-call rota. Working arrangements currently consist of the following:

Scheduled evening post-take on call duties 5pm 8pm Monday to Friday currently 1:10 seeing older patients (> 80yrs or > 65yrs from Care Homes or patients with frailty) admitted via the Acute Care of the Elderly Unit varies from one or two patients to five/six patients

On-call weekends currently 1:8 8-hour resident on call seeing approximately 20-25 patients with the support of the registrar. 1 consultant on call at Dewsbury and District hospital and 2 at Pinderfields General Hospital.

Overnight On-call duties are currently undertaken on a 1:18 currently.

The on-call element of a 10PA job plan is assessed as 1.5PA. Those on less than full time working have on call commitments depending on their preference of 75% or 100%.

Supporting Professional Activities in accordance with the Trust Framework, with at least a minimum of 1.5 SPAs included in the core 10PA timetable. Additional SPA or DCC time may be commissioned in line with the skills of the successful candidate and changing requirements of the service.Examples may include formal teaching responsibilities or departmental leadership roles

Secretarial Support and Offices

Each consultant is designated an office space within the office suite at Pinderfields Hospital or the suite of offices at Dewsbury and District Hospital. All the Elderly Medicine Consultant offices are located near the secretarial support which is available at both sites. Each consultant has a designated secretary/secretary (some job share). Every consultant has access to either a desktop or laptop depending on need for remote access.

Newly appointed senior medical staff will be offered mentoring opportunities either within the department or within the wider Trust and may be specified by the appointments committee. There is an induction and development programme for all new staff. The Trusts has an ongoing commitment of support and training and mentoring or coaching is available for all not just new staff.

Continuing Professional Development

The Trust requires all its doctors to engage with continuing professional development, guided by the relevant professional associations or Royal Colleges. Time and funding is provided according to departmental and Trust policies, subject to approval through the clinical management structures. There is a programme of mandatory training which all relevant staff must complete as required and the department organises its own education programme throughout the year and there is a Grand Round for the medical specialties once a week.

Teaching

The Trust is keen to maintain its reputation for high quality training and to attract the highest calibre of trainee and career-grade doctors. All senior doctors are involved in training our doctors, students and other professions. There are many opportunities to develop training and education roles and many have taken on educational leadership roles within and beyond the Trust.

Research

Medical staff are encouraged to develop areas of personal and/or research interest. This can be developed in liaison with the Trusts R&D department and in line with prevailing policies and research frameworks. The Trust is developing an expanding and outward-looking research programme.

Quality Improvement, Audit and Clinical Governance

The Trust has its own bespoke Quality improvement programme which enables clinicians, managers and other members of staff to work collaboratively and strive to improve patients experience and outcomes. There is a rolling half-day clinical governance programme and audit meetings are well attended and lively, as well as a commitment to completion of Structured Judgements and investigations where incident occur. The Clinical Audit Department assists with setting and supporting an audit programme for the Service with which all colleagues are encouraged to engage. Clinical Audit is considered within objectives as part of job planning as is quality improvement work.

All our senior doctors will engage positively with appraisal and revalidation. The have a precribed connection with Mid Yorkshire Teaching NHS Trust via GMC Connect and their Responsible Officer is the MYTT Chief Medical Officer. Job Planning takes place with our doctors and clinical teams on an annual cycle; our Job Planning Framework is used as a best-practice model by other organisations.

Representation

Our Senior Medical Staff Committee provides a forum to liaise with and advise our senior management team. The BMA-accredited Local Negotiating Committee negotiates on terms-and-conditions matters.

General Conditions

The post is advertised on a full-time basis (although less than full time applicants will be considered) and on the basis of the prevailing national TCS and pay scales for this grade. These may be obtained from the NHS Employers website or provided upon request as well as details of joining the NHS Pension Scheme.

The appointment is subject to satisfactory medical health clearance from the Trusts Occupational Health Department. The successful applicant may be required to undergo an examination prior to taking up appointment. The post is subject to an enhanced DBS clearance evidenced by the applicant.

When on duty the postholder should remain contactable and able to reach attend the relevant clinical site within 30 minutes in normal traffic conditions (or more immediately available should clinical circumstances demand).
