Consultant care of the elderly

This post will enable the post holder to have some flexibility on their job plan by contributing to the acute DMOP wards, in reach liaison service, and Frailty Assessment Unit.

The post holder will have significant independence in developing the job and will be encouraged to be innovative and responsive in changing the job plan to meet service needs. The department is keen to move elderly services high on the agenda and is looking to recruit consultants with an appetite for improvement.

Main duties of the job

Clinical Duties:

The delivery of a dedicated programme of geriatric care.

Assume responsibility for patients on Elderly Medicine wards with daily review and full consultant ward rounds.

Regular pro-active review of our patients and coordination of care with allied specialties.

Engagement in multi-disciplinary meetings to direct the care of patients.

Provide specialist opinions to other hospital specialties as part of frailty liaison service.

Participation in outpatient clinics.

Supporting the delivery of patient care within Geriatric Medicine in line with national standards and Care Quality Commission requirements.

Active participation in developing innovative approach to acute elderly care.

Developing older peoples acute service pathways.

Developing older people services / Outpatient clinic.

To facilitate the further development of our Frailty Assessment Unit

Actively participate in clinical audit and quality improvement initiatives.
