A London-based healthcare provider seeks a dedicated nurse for the Barnet Children's Continuing Care Team. This rewarding role involves assessing children's eligibility for NHS care packages while working closely with families and multidisciplinary teams. The ideal candidate must possess relevant nursing qualifications (RGN, RSCN, RN Child, or RN LD) and demonstrate strong clinical skills, ensuring responsive and personalized care for children with complex health needs. The Barnet Children's Continuing Care Team is a small, supportive, and highly committed team of very experienced nurses responsible for coordinating and overseeing NHS Funded Continuing Care packages for children and young people with complex and long-term health needs.
Working in line with the National Framework for Children and Young People's Continuing Care (2016), the team ensures that children and young people receive safe, high-quality, and personalised care that enables them to live as well as possible within their homes and communities.
At the heart of this role is partnership working. You will work closely with children, young people, and their families, as well as with health, social care, education, and voluntary sector partners across the London Borough of Barnet, to ensure that commissioned packages of care are responsive, effective, and centred on individual needs.
