Job description

The Barnet Children's Continuing Care Team is a small, supportive, and highly committed team of very experienced nurses responsible for coordinating and overseeing NHS Funded Continuing Care packages for children and young people with complex and long-term health needs.

Working in line with the National Framework for Children and Young People's Continuing Care (2016), the team ensures that children and young people receive safe, high-quality, and personalised care that enables them to live as well as possible within their homes and communities.

At the heart of this role is partnership working. You will work closely with children, young people, and their families, as well as with health, social care, education, and voluntary sector partners across the London Borough of Barnet, to ensure that commissioned packages of care are responsive, effective, and centred on individual needs.

Main duties of the job

As part of the Barnet Children's Continuing Care Team, you will:

  • Undertake Children's Continuing Care assessments and reviews to determine eligibility for NHS funded Continuing Care packages
  • Work with Children and Young people who may have life-limiting or life-threatening conditions, and or be technology dependent
  • Provide ongoing case management for children and families, ensuring care packages remain safe, effective, and appropriate to changing needs
  • Act as a key point of contact for children and families within your caseload, offering support, coordination, and guidance
  • Liaise closely with multidisciplinary teams, community services, and partner agencies, working across organisational and professional boundaries
  • Manage care packages through to transition into adult services, or until continuing care is no longer required
  • Take appropriate reporting action regarding any accident or incident to a child, family member, staff member or visitor this include Safeguarding.
  • Demonstrate transferable skills, underpinned by relevant nursing experiences.
  • Ability to consistently demonstrate the 6 C's of Nursing in practice.
