If your ambition is to make a difference - please apply. We have an opportunity for a permanent Registered Nurse to join our friendly Overnight Community Nursing team within York and Scarborough Teaching Hospitals NHS Foundation Trust Adult Community Services.

Responsibilities
- The Overnight Community Nursing team is comprised of motivated professionals who work out of the hospital setting delivering generally unplanned holistic nursing care in patient\'s homes and other community settings.
- We have close working networks with our hospital, social and primary care colleagues to support patients to stay in their homes and avoid hospital admission.
- The team also work closely with the hospice at home team and our palliative care support team. The Out of Hours GPs are based at York Hospital and support is also available from the hospital Out of Hours management team.
- Main duties of the job: The team serves a large geographical area including the City of York, Selby, Easingwold and surrounding villages, Pickering, Kirkbymoorside and surrounding area and are based at White Cross Court in York.
- The team use a buddy system during the overnight period consisting of a community staff nurse supported by a health care support worker.
- The shifts are from 19.45 until 07.45 with a one hour unpaid break.
- Pool cars are provided for the overnight team and it is essential that you are a car driver. It is essential that you have the ability to transport yourself and equipment across the locality during the course of a normal working day. Travel expenses, if incurred, will be covered by expenses and parking at our community sites is free for clinical staff.
