Overview

The Out of Hours Community Nursing Team across Cambridgeshire is currently offering opportunities for permanent contracts working full time and part time within CPFT across the county working in Cambridge, Huntingdon and Peterborough. We provide a service 7 days a week, working closely with other community services and PCNs to provide high standard holistic care to the population of Cambridgeshire.

Cambridgeshire & Peterborough NHS Foundation Trust is a health and social care organisation dedicated to providing high quality care with compassion to improve the health and wellbeing of the people we care for.

All staff will be expected to work the following shift patterns over a 7 day period: Twilight shift 18:00–24:00; Night shift 21:45–08:15.

Key Responsibilities
  • Provide high standard of nursing care and interventions within the patient’s own home, with support from team leaders.
  • Ensure patient and carer participation in decision making and keep the patient at the centre of care.
  • Accountable for assessing, interpreting, planning, implementing and evaluating treatment to patients within professional guidelines and using evidence‑based practice.
  • Identify patient needs through holistic nursing and promote independence to avoid hospital admission.
  • Contribute to risk assessments and health and safety assessments and report any changes or new risks.
  • Effectively communicate verbally with patients, carers and colleagues using tact and persuasive skills and adapt communication to patients with hearing loss, diminished sight, depression, speech problems, cognitive impairment or behavioural problems.
  • Work effectively within the Out of Hours Team structure, liaise with the MDT and attend MDT meetings and case reviews.
  • Ensure accurate and timely written records are kept in accordance with Trust policy and NMC guidance.
  • Provide support, education and mentoring to peers, new staff and students.
Qualifications & Experience
  • Registered nurse and currently registered with the Nursing and Midwifery Council.
  • Experience working in community or out‑of‑hours nursing settings is desirable.
  • Full UK driving licence and personal vehicle for independent travel around the county; public transport is not acceptable.
  • Demonstrated ability to work autonomously and accountable for professional practice within the Out of Hours Countywide team.
  • Professional, legal and NMC accountability for delegated and independent work.
