We want you:

If you have a passion for rehabilitation, thrive working within multidisciplinary teams and have innovative ideas to develop and deliver personalised care plans that support people to maintain their health and wellbeing, to promote hospital admission avoidance. You will need excellent communication skills and commitment to high quality patient care alongside flexibility and resilience to meet the challenges of community working. Opportunities exist for you to work with rapid access from our referral centre, Home First' and domiciliary based intervention, so there's plenty of scope for your professional development.

We will give you:

A chance to work across adult integrated health and social care services, supported by Clinical specialists, Advanced Practitioners and Professional Leads, to help you grow and develop your professional and clinical skills in a variety of areas, so that you can achieve competence and specialisms in your particular field of interest.

As a B5 you will need:
