Your Healthcare (YH) is a CommunityInterest Company (CIC) located in the South West of London within the RoyalBorough of Kingston-upon-Thames and provides Learning Disability Services toboth Kingston and Richmond.

An excitingopportunity has arisen to recruit to the post of band 5 Community Learning DisabilityNurse to join Your Healthcares Neurodevelopmental Service. We would welcomeapplications from registered nurses who have a special interest in working in learningdisability services or those approaching the completion of their Nurse training.This team is ideally equipped to support you develop your skills and knowledgein this field.

The Neurodevelopmental Service provides a comprehensiverange of specialist community services for adults with learning disabilitieswithin a multidisciplinary healthcare team. We also provide assessment and interventionservices for adults with Autism Spectrum Conditions (ASCs), Attention DeficitHyperactivity Disorder (ADHD).

Main duties of the job

The community learning disability nursing team is an integral part ofthe neurodevelopmental services multi-disciplinary team facilitating andsupporting a wide range of health interventions including specialist healthassessments, transition work from child to adult services, Health Plans, Healthpromotion and early intervention, Primaryand Secondary Care liaison and support, epilepsy management, dementia care, endof life support, individual bespoke pathways, patient led care and much more.

In additions to working with our NDS multi-disciplinary team we workclosely with other services within YH such as District Nursing, Rapid Response tocoordinate holistic care and support our nurses to increase their skills andknowledge.
