Job Summary

Are you a Registered Nurse who is passionate about delivering high quality care? Do you have an interest in supporting the health and well-being of people within our communities? We have several opportunities for passionate, motivated Registered Nurses to join our Community Nursing Teams, working autonomously to provide a wide range of clinical and care interventions across a range of community settings.

We have various hours available, working across a 7 day period within a number of teams including Haverfordwest, Milford Haven, Pembroke Dock, Tenby and Narberth.

We offer a supportive induction with an ongoing focus on training and development.

Interviews will be held face to face.

Please note these posts cover large rural geographical areas and the ability to travel between settings in a timely manner is essential.

Main duties of the job

Be flexible and adaptable in order to accommodate the needs of the service, be able to contribute to the performance of the community nursing team and develop professionally within the role.

Assessment of nursing needs, deciding on the use of appropriate assessment tools, to plan appropriate nursing care, deciding on frequency of visits, deciding when care should be reviewed, altering care plan accordingly, in conjunction with the District Nurse Team Leader. This will ensure successful outcome for appropriate care designed and adapted to meet individual need. All care provision based on robust evidence based clinical research.

Work alone and with confidence to make decisions without supervision. Work in line with the Health Board Lone Worker policy.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
