An exciting opportunity has come up for the Adult Community Nursing Service. We are currently looking for a Band 5 Community Nurse to work across the Hounslow areas based at the Heart of Hounslow Health Centre. The service operates from 07:00 until 19:00 seven days a week, working alternate weekends, comprising three shifts: 07:00‑15:00 (early), 08:30‑16:30 (day) and 11:00‑19:00 (late).

Collaboratively working with our integrated rapid response teams, community matrons and social service colleagues is essential to ensure seamless care to our patient group.

To provide high‑quality, skilled nursing care and to assist the Community Nursing Team Manager in the ongoing planning, implementation, and evaluation of the holistic needs of the patient/client, family, and/or carer.

Deliver high‑quality nursing care to housebound patients with complex needs to enable them to remain in their own homes. Working closely with specialist nurses, allied health and social care professionals to meet the needs of the patients without unnecessary attendance in A&E or admission to hospital.

This will involve working independently but as part of the Community Nursing Team covering a particular geographical area. This will include first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care. The candidate will be expected to be flexible to support corporate approach to patient care as and when required.
