Community Nurse within Patient Care in Isle of Wight (UK), earning £21.00 hourly for a 6-month period.

What you will do
  • Work as an integral member of the Integrated Community Nursing Team providing comprehensive nursing care for patients in their own community settings.
  • Promote a positive image of community nursing and deliver an exceptional service to all patients.
  • Maintain patient confidentiality and adhere to data protection regulations while accurately managing both written and electronic records.
  • Communicate effectively with patients' families and carers, providing necessary support and guidance in collaboration with the District Nurse Team Leader.
  • Instigate, participate in, and evaluate care plans as part of an Integrated Locality Service, maintaining high standards of patient records.
  • Ensure the provision and safe use of required nursing equipment, instructing patients and their families on proper use.
  • Engage in informal teaching for patients and carers as well as assisting in the induction and training of new staff.
  • Seek opportunities to rotate through the Emergency Department and Medical Assessment Unit, as guided by the District Nurse Team Leader.
