A healthcare organization in Greater Manchester is seeking a part-time Community Mental Health Practitioner to provide mental health services for adults. The candidate should be a registered mental nurse or social worker, committed to community health and capable of managing patient care. This position entails collaboration with multi-disciplinary teams and focuses on a needs-based approach to service delivery.
We currently have a vacancy for a part time Community Mental Health Practitioner within our CMHT. The post is based at Horton House, Hamilton Street Oldham.
You will need to be a registered and experienced RMN, or Social Worker to apply for this post.
Service/Ward/Department Summary: The CMHT work primarily with working age adults who have a diagnosis of a Serious Mental Illness and / or Complex Mental Health Difficulties. The care pathway includes a holistic assessment of need / risk, formulation and delivery of a care and treatment plan with a focus on recovery and supporting individuals to achieve and maintain their optimal level of functioning.
