Job description
Community Mental Health Practitioner

The Tower Hamlets Stepney and Wapping Neighbourhood Mental Health Team is a well‑established team within East London Foundation Trust. The team operates as an integrated multi‑disciplinary team, with staff from both the East London NHS Foundation Trust and the London Borough of Tower Hamlets working together to provide a high‑quality service for people with severe and enduring mental health needs. This work is aligned with the Transformation Project and is carried out in partnership with service users, carers, the Local Authority, statutory services and local voluntary groups.

Responsibilities
  • Take responsibility for a caseload and apply the agreed model of care as set out in the operational policy.
  • Conduct assessment, plan, implement and evaluate care, including assessment under the I Care Act.
  • Administer prescribed medication, including intramuscular injections, following Medicines Management Guidance and associated policies.
  • Develop and maintain a high standard of practice through psychoeducation, the Recovery Model and supportive clinical management.
  • Establish and maintain good working relationships with referring agents, Primary Care, Adult Social Care, other neighbourhood teams, in‑patient services, Criminal Justice, Voluntary Sector and other agencies.
  • Work autonomously within the MDT and collaborate in developing plans that offer supportive crisis care in the least restrictive way.
  • Ensure compliance with Safeguarding Adults and Children’s policies.
  • Promote advocacy, autonomy and self‑empowerment, ensuring the team adheres to these principles.
  • Understand the KPIs for the service and participate in improving team quality and performance.
  • Identify own educational needs and pursue professional updating and development as appropriate.
  • Maximise potential through supervision, appraisals and performance management.
  • Address the physical health needs of service users.
